OFFICIAL RULES AND REGULATIONS AND AFFIDAVIT OF ELLIGIBILITY AND RELEASE

INDEMNIFICATION, WAIVER, RELEASE
AND HOLD HARMLESS AGREEMENT

Any and All contestants of any Contest agree to and are bound by the following rules and regulations.

The undersigned winner of the following prize, as a condition of receipt of the prize (the “Prize”) – and in consideration therefore, hereby agrees, acknowledges and represents, on behalf of himself/herself and on behalf of his/her heirs, successors and assigns, as follows:

1. I am 18 years of age or older.

2. I am a legal resident of the United States.

3. I understand and acknowledge that I am responsible for all federal, state or local taxes and any other expenses associated with my receipt or use of the Prize.

4. I understand and acknowledge that my right to claim the Prize is not transferable, and that the Prize may not be substituted or redeemed for cash value.

5. To the fullest extent that I may lawfully do so, I hereby waive and relinquish any rights and benefits that I have or may have under any statute, law or rule to the effect that a general release does not extend to claims which a creditor does not know or suspect to exist in his or her favor at the time of executing such release. I hereby acknowledge that I am aware that I may hereafter discover facts in addition to or different from those which I now know or believe to be true with respect to the matters specifically or generally referred to as released in this document, but it is my intention to fully settle and release any and all matters, disputes and differences, known and unknown, suspected and unsuspected, with respect to each and every matter specifically or generally referred to as released in this document, notwithstanding discovery or existence of any such additional or different facts.
